HOW TO REGISTER
There are three easy ways to register for MMDA’s Annual Conference
1. Register online at http://www.marylandmda.org/registration/
2. Register by Fax by printing the registration form here and faxing the completed form to 410-975-4625
3. Register by mail by printing the registration form here and returning it with payment to MMDA, c/o Ms. Catherine Johannesen, 1211 Cathedral Street, Baltimore, MD 21201
All cancellations must be made in writing to the MMDA Registrar, Ms. Cathy Johannesen. Notices can be emailed to mmdawebsite@gmail.com or faxed to 410-975-4625, Attention: Registrar. Cancellations received by October 29, 2011 will receive a full refund less a $50.00 administrative fee. Cancellations received after October 29, 2011 or up to November 4, 2011 will be refunded half of the registration fee. Fees will be refunded the same way they were paid to MMDA. Please allow up to four weeks for delivery of refund if payment was made by check. Cancellations received after November 4, 2011, will result in the forfeiture of all registration fees. No-Shows will also forfeit all registration fees. Partial or full fees that have been forfeited cannot be applied to subsequent meetings.

Reservations
To make your room reservation, please call 1-800-526-2593 and mention that you are with the MMDA meeting in Annapolis. To guarantee room rate and availability, you must make your reservation no later than Tuesday, October 11, 2011, 11:59 p.m. All reservations must be accompanied by a valid credit card.

Event parking: Self-parking - $6 per vehicle per day
