MMDA 2011 Conference Registration

Register for MMDA's 2011 Conference online.  Complete the registration form below then click on the the submit button.  You will then be directed to our secure online payment system using PayPal.  To reserve a room, click this link to the Loews Annapolis Hotel and be sure to reserve the room no later than October 11 to obtain the reduced rate.    

If you choose to mail a check to MMDA, please complete the form below then click the submit button.  Please mail your check to MMDA, C/O MedChi, 1211 Cathedral Street, Baltimore, MD 21201.  Alternatively, click here to download a printable registration form and payment instructions.

This year MMDA is reducing our carbon footprint.  HANDOUTS WILL BE AVAILABLE FOR DOWNLOAD ONLINE ONLY AND WILL NOT BE SUPPLIED ONSITE.  Check your confirmation notification or e-mail us at mmdawebsite@gmail.com for the download passcodes (please allow 24 hours for response.)

CANCELLATION POLICY FOR MEETING REGISTRATION FEES:
All cancellations must be made in writing to the MMDA Registrar, Ms. Cathy Johannesen.  Notices can be emailed to mmdawebsite@gmail.com or faxed to 410-975-4625, Attention: Registrar.  Cancellations received by October 29, 2011, will receive a full refund less a $50.00 administrative fee. Cancellations received after October 29, 2011 or up to November 4, 2011 will be refunded half of the registration fee.  Fees will be refunded the same way they were paid to MMDA.  Please allow up to four weeks for delivery of refund if payment was made by check.  Cancellations received after November 4, 2011, will result in the forfeiture of all registration fees. No-Shows will also forfeit all registration fees.  Partial or full fees that have been forfeited cannot be applied to subsequent meetings.
